Your First Chiropractic Visit
In preparation for your Chiropractic appointment each person will be asked to fill out forms. You can fill the forms out in the office at the time of your appointment, or click the button below to download a printable, and digitally fillable pdf. You may print these at home and bring them with you to save time on the day of your appointment. This will provide all the information we require concerning your health history. Please do not email any Protected Health Information (PHI).

What to bring
We ask that you provide your insurance card and medication list so that we may have a photocopy for our records.
Your first Chiropractic exam visit will last about 80 minutes.
A thorough chiropractic exam will be performed which includes taking your vitals, range of motion testing, nerve testing, muscle strength, palpatory tenderness, and orthopedic examination. All of this helps your doctor determine the cause of your condition and the best possible treatment plan. You will also go over your health history with the doctor and he will take x-rays if he feels it is necessary. If you have past MRI's, X-Rays or other imaging we can use these as diagnostic tools as well. After all testing is finished you will receive a chiropractic adjustment and the doctor will go over a treatment plan specific to your needs.

We work very hard to make sure you are scheduled at a time that works with your busy schedule. Your appointment will last between 10 and 20 minutes after you are an established patient.
We will be more than happy to submit all insurance forms for you to help you recover the most from your benefits. We will submit billing for Medicare, General Insurance, Secondary Insurance, Workers Compensation and Personal Injury Cases. We will work with you and do whatever we can so that you may afford all the treatment you want and need. Forms of payment accepted are cash, check and credit cards. Credit Card transactions will be charged a 3.5% processing fee.
If you are unable to keep an appointment, we kindly ask you give us 24 hours notice. We do understand situations do arise that will make you unable to give us appropriate notice, instead please just let us know as soon as possible so we can offer this appointment to another patient.
As of April 2013, our "No Show" Policy has been updated to help not only our patients but also our staff.
If you incur 3 of the following in a calendar year:
1. "No Show" for a scheduled appointment
2. Arrive late for a scheduled appointment, or
3. Call less than 30 minutes prior to a scheduled appointment
You will no longer be able to reserve time on the Doctor's schedule. You will be eligible for "same-day" appointments only.
